1.-Q: We do not register neither height nor weight of our patients; what shall we do?
A: Fill the corresponding cells with the number 0 (zero)

2.-Q: We do not register the date of birth, however we do register the age of the patient
A: You can fill with the date 1st June of the corresponding year

3.-Q: What shall we do in the case of an heterotopic pregnancy i.e. an ectopic coinciding with a normotopic pregnancy?
A: Please register the outcome of the normotopic pregnancy, and in the field “comments” add the corresponding explanatory note

4.-Q: What shall we do in the cases where a pure-donor, donates to two or more oocyte-receptors?
A: Register each cycle of each oocyte-receptor separetely

5.-Q: How can we upload an EXCEL file?
A: Previously, we have provided your centre with an example of an EXCEL file with examples. Please follow carefuly that example. It is very important to write exactly the same way as in the provided example, with the same spelling and spaces, so the system can recognise your file.

6.-Q: What is chart number
A: It is an unique number that identifies the patient. It can be a personal identification number, hospital chart number, or correlative number

7.-Q: May we live the chart number cell blank?
A: No, you may not. In the case you do not register a personal identification number, you can write a correlative number

8.-Q: What is initial date?
A: It corresponds to the first cycle-related drug administration.
Per example, your patient undergoes a regular IVF cycle. She starts with agonist the 28th of october, and rFSH the 5th of November, and undergoes oocyte pick up the 17th of November. Her initial date is the 28th October.

9.-Q: How do we register cases of IVF/ICSI and PICSI?
A: Please register them as ICSI. In the case of PICSI please add a note in the field “comments”

10.-Q: How do we register cases in which the patient decides to fertilize some oocytes and cryopreserve the rest?
A: Those correspond to regular IVF/ICSI cycles.

11.-Q: What diagnose should we choose, when the reason to undergo IVF/ICSI is advanced maternal age
A: You should use “other”
